Instructions for completing the CITI IRB Ethics training and the HIPPA Training
(only needed once every three years)
CITI Training
Step 1: Download these step-by-step instructions and then register for an account at CITI.
Step 2: Complete the two CITI modules listed below:
1. The Responsible Conduct of Research
2. Human Subjects Protection*
*The dialogue box for choosing this module says "Yes, I want to complete the course in Lab Animal Welfare or the course in the Protection of Human Subjects", so it's easy to miss. You do not need to complete the Animal Welfare module. However, you do need to complete the Human Subjects Protection module.
Step 3: Save your two CITI training completion reports as PDF files with your first and last name in the file names.
Step 4: Upload both completion reports by completing the form below and clicking “submit”

Step 6: Review the SBU Policy and Procedure on Research Subjects' Right to Privacy here. See Section 16 of the Standard Operating Procedures.
Step 7: Review the University Hospital's general HIPAA awareness training materials here.
Step 8: After reviewing both above, send an e-mail to Mary Ellen Herz (maryellen.herz@stonybrook.edu)
Subject line: HIPAA research training completed
Email text: "I have read and understood the HIPAA awareness training materials and agree
to comply with the SBU Policy and Procedures on Research Subjects' Right to Privacy."
Include your first and last name at the bottom of the email.
Underneath your name, write one of the following:
Undergraduate LC Research Lab Assistant for Dr. Lily Cushenbery
Graduate LC Research Lab Assistant for Dr. Lily Cushenbery
Step 9: Congratulate yourself on finally becoming an ethical person.
